Summary:
The purpose of this research project was to evaluate a patient assessment tool based on Prochaska and DiClementi's Transtheoretical Model (Prochaska & DiClemente, 1982) and Bandura's Self-efficacy Theory (Bandura, 1995). The tool was designed to be used by primary care providers (PCPs) to assess selected patients' individual stage of change, barriers to change, and self-efficacy for change for diet and exercise behaviors. Twelve PCPs were recruited to participate in the evaluation. The PCPs were asked to use the as s thesessment tool with a minimum of 20 patients and evaluate the usefulness of the tool using an instrument that included a 3-point scale. The investigator retrieved the evaluations from the PCPs in person or provided self-addressed stamped envelopes for that purpose. Seven PCPs completed the project. The tool was given a mean score of 2.46 by the PCPs on a scale of 1 to 3. Six of the PCPs gave the tool a mean score of 2.5 or higher. One PCP gave the tool a mean score of 1.5. A standard approach using the newly developed assessment tool allows PCPs to tailor interventions to each patient's stage of change and and address the barriers each patient faces.
