Approximately 500,000 patients undergo coronary artery bypass grafting (CABG) each year, 20% of whom have diabetes mellitus" (Chitwood, Estrada, Nifong, & Young 2003 p.1392). The average incidence of deep sternal wound infections (DSWI) in a diabetic patient is two to five times that of someone without diabetes (Furnary, Grunkemeier, Starr, & Zerr 1999). These statistics have led researchers to continue their search in how to optimize healing and minimize the incidence of DSWI and post operative healing and minimize the incidence of DSWI and post operative complications. A new insulin protocol was developed after an extensive literature search in the management of hyperglycemia with the use of a continuous insulin infusion protocol in the post operative CABG patient. Proper instrumentation and implementation of insulin protocols stem from the proper training of those nurses and physicians involved in the deliverance of the protocol. A web-based inservice with pre and post testing was developed to educate staff prior to the implementation of the insulin protocol.
